2. Fill out the form on the computer, then print it; or print it out to complete by hand.
3. Mail the form to the address at the bottom of the complaint form. Include copies (DO NOT SEND ORIGINALS) of any documents that are relevant to your complaint, such as receipts, contracts, leases, repair orders or sales agreements. You may wish to keep a copy of the complaint for your records.
